How do I get started in the Pulmonary Rehab program?

To enroll into our individualized Pulmonary Rehabilitation program, you will need a referral for Pulmonary Rehabilitation from your physician. We would be happy to assist you in obtaining that referral.

Click on this link to our Available Forms page, download for the Little Rock or Hot Springs location, print and take to your doctor.  Your doctor will then fax it to us; we usually call within a week to schedule a consultation to determine what program is best for you
